Case Manager - Young Adult Services

Description
Two full-time openings. Work as part of a multidisciplinary teamin our Young Adult Services (YAS) Program. Provide case management and housing services for young adults, ages 18-25, who have a diagnosed mental illness and are in need of community-based support services in the Willimantic area.If you have knowledge and experience with substance abuse or community support services for young adults and an understanding of the developmental issues of this population, please consider becoming part of our team.YAS provides a diverse array of services including clinical, medication management, case management, vocational, recreational, transitional living and housing services. Schedule: Monday through Friday 9:00 a.m. – 5:30 p.m.
Minimum Requirements
Bachelors degree in a relevant human services field.
Salary Range:
- $14.26 - $16.13 per hour depending on experience.
