Case Management Services

Case Management staff helps individuals who have severe and prolonged mental illness to live independently in the community. Family and friends are integral to a person's well being and with a client's invitation may become significant contributors to the service planning process.

Case Management Services include:

  • Assessment of needs

  • Service planning, coordination and monitoring

  • Linkages to community services

  • Advocacy and support

  • Obtaining and maintaining financial benefits

  • Assistance in finding and maintaining affordable housing

 

Eligibility: Meets eligibility criteria of severe and prolonged mental illness as defined by the Department of Mental Health and Addiction Services.

Funding is provided by the Department of Mental Health and Addiction Services.
