Case Manager (COAST)

Description:

United Services offers an exciting opportunity to work with the COAST (Community Outreach Assistance Support Team) program and is seeking passionate, dedicated case managers to support individuals who need multiple touchpoints throughout the week to remain stable in the community. The right candidate will coordinate care with providers and work closely with individuals on their caseload, collaborating to meet people where they are so their recovery and overall wellness can be strengthened.

As a case manager, you have a unique opportunity to build a therapeutic connection with adults who have been diagnosed with mental health and substance use disorders. When individuals are asked what made the difference in their lives, the most frequent response is that they had someone who believed in them and gave them hope.

Through this work, you can help individuals achieve what everyone deserves: safe and stable housing, access to quality mental health and medical care, meaningful personal relationships, productive activities including employment, and a strong connection to the community in which they live.

Schedule:

Monday – Friday 8:00 a.m. – 4:30 p.m. with flexibility, no evening or weekends

Minimum Requirements:

Associate’s Degree in relevant human services field plus two years of relevant experience or Bachelor’s Degree in relevant human services field.

Salary Range:
  • Salary/Hourly Range: $19.00- $21.50  per hour depending on experience
  • Bilingual Spanish/English Fluency: $19.96- $22.59 per hour depending on experience
