In this role, you will support individuals living with mental health and substance use disorders who benefit from frequent, community-based support to maintain stability and continue their recovery journey. Working collaboratively with providers, community resources, and each individual on your caseload, you will help people access the services and supports they need to thrive.<\/p>\n<p>*Spend up to 70% less time on paperwork with documentation support technology designed to help you focus more on care.<\/p>\n<p>Location: 140 North Frontage Road, Mansfield Center, CT 06250<\/p>\n<p>What You Will Do (Day-to Day): <\/p>\n<p>\u2022\tProvide intensive case management services to adults with mental health and substance use disorders.<\/p>\n<p>\u2022\tBuild trusting, relationships that promote recovery and personal growth.<\/p>\n<p>\u2022\tCoordinate care with healthcare providers, community agencies, and support networks.<\/p>\n<p>\u2022\tConduct regular community-based visits and maintain frequent contact with individuals on your caseload.<\/p>\n<p>\u2022\tAssist individuals in accessing housing, healthcare, employment opportunities, benefits, and other community resources.<\/p>\n<p>\u2022\tCollaborate with individuals to establish goals and develop plans that support their recovery and wellness.<\/p>\n<p>\u2022\tAdvocate for individuals and help remove barriers to achieving greater independence and stability.<\/p>\n<p>\u2022\tDocument services and maintain accurate records in accordance with program and regulatory requirements.<\/p>\n<p>Schedule:<\/p>\n<p>Monday \u2013 Friday 8:00 a.m. \u2013 4:30 p.m. with flexibility, no evening or weekends<\/p>\n<p>Salary\/Hourly Range:<\/p>\n<p>$19.00- $21.50 per hour depending on experience<\/p>\n<p>Bilingual Spanish\/English Fluency: $19.96- $22.59 per hour (depending on experience)<\/p>\n<p>Minimum Requirements:<\/p>\n<p>Associate\u2019s Degree in relevant human services field plus two years of relevant experience or Bachelor\u2019s Degree in relevant human services field.<\/p>\n<p>Benefits Package: <\/p>\n<p>\u2022\tGenerous comprehensive benefit package for medical, dental, and vision coverage for the employee and their family<\/p>\n<p>\u2022\tLife, AD &amp; D, short-term and long-term disability insurance plans \u2013 100% employer paid<\/p>\n<p>\u2022\tFour weeks of paid vacation<\/p>\n<p>\u2022\t12 paid holidays<\/p>\n<p>\u2022\tPaid Sick time<\/p>\n<p>\u2022\tFlexible Spending Accounts for healthcare and dependent care reimbursement<\/p>\n<p>\u2022\tProfessional Development Funds<\/p>\n<p>\u2022\t403B Thrift Plan (tax-deferred) with employer contribution and match after one year of employment<\/p>\n<p>\u2022\tEmployee Assistance Program<\/p>\n<p>\u2022\tMileage Reimbursement<\/p>\n<p>\u2022\tUnited Services is a designated employer for the National Health Services Corps loan repayment and scholarship program<\/p>\n<p>*Benefits will be pro-rated for eligible part-time positions<\/p>\n<p>Work Location: Because our clients relay on face-to face support to build trust, this position requires employees to be fully present in the community and in the office.<\/p>\n<p>Important Employment Requirement:<\/p>\n<p>All employees of United Services, Inc. are required to receive the influenza (flu) vaccine each year, unless a medical exemption has been submitted and approved by our Medical Director.<\/p>\n<p>Join Our Team!<\/p>\n<p>Do you want to make a direct impact in the lives of children, families and adults with social and behavioral health needs?
