Description:

United Services is looking to hire full-time Nurse Care Manager (LPN/RN). If you are passionate about working with individuals with behavioral health diagnoses and chronic health conditions with a team approach and are looking for a supportive work environment that encourages a good work-life balance, we are looking for you!

The Nurse Care Manager will provide care management, care coordination, individual and family support, hospital transition planning and health promotion services to individuals that receive services.

Schedule:

Monday-Friday 8:30am-5pm with no evening, weekend or holidays required.

Minimum Requirements:

Current Registered Nurse (RN) / Licensed Practical Nurse (LPN) license. Able to maintain and renew licensure as RN/LPN in the state of Connecticut.
