Description:

United Services is looking to hire full-time Nurse Care Manager (LPN/RN). The Nurse Care Manager will provide care management, care coordination, individual and family support, hospital transition planning and health promotion services to individuals that receive services.

Schedule:

Monday-Friday 8:30am-5pm with no evening, weekend or holidays required.

Minimum Requirements:

Current Registered Nurse (RN) / Licensed Practical Nurse (LPN) license. Able to maintain and renew licensure as RN/LPN in the state of Connecticut.
