Integrated Primary Care provides comprehensive medical care services. Staff includes board-certified APRNs, Nurse Care Managers and Care Coordination staff and medical assistant/phlebotomists. Primary Care is currently only available to individuals who are currently receiving services at United Services.

Services include assessment, diagnosis and treatment of illness, complete physical examinations, women’s health care/GYN, promotion, chronic disease management, specialty referrals, and immunizations. On-site testing including EKG, rapid point of care testing (blood glucose, Strep, Covid, flu, HIV, mononucleosis, Hepatitis C, pregnancy, lipids) and referral for diagnostic testing. Onsite blood draws available.

The Primary Care Program participates in the CT Breast and Cervical Cancer Early Detection Program. Women without insurance may receive a physical, Pap smear and mammogram at no charge.

The Primary Care Program is currently only located at United Services Dayville clinic, 1007 North Main Street, Dayville. Expansion to the agency’s Mansfield Center clinic is planned for later in 2024.

What to bring for your first visit

Please bring your insurance card, documentation of income (2 pay stubs) for participation in the sliding fee scale, and a list of your current medications. You may also bring records from your previous physician if applicable. If there is a guardianship the authorized individual is responsible for signing consent forms.

Insurance info

United Services accepts most insurance plans. USI staff obtains health insurance authorization and reauthorizations as needed. Staff assistance is available for people interested in applying for Medicaid, HUSKY or Healthy Start.
