Description:
United Services is looking to hire a full-time Care Navigator in our Comprehensive Services Division. Comprehensive Services’ goal is to help transform community behavioral health systems and provide comprehensive, coordinated behavioral health care and medical care. The focus is on integrating care throughout the behavioral and medical health system to benefit individuals meet their recovery goals.

This is a great opportunity to work with individuals to meet their health and wellness goals, provide person-centered care, and provide care coordination to have a positive impact on one’s life. Care Navigators will provide care management, care coordination, individual and family support, hospital transition planning and health promotion services to those that are enrolled in Comprehensive Services.
The Care Navigator coordinates the person centered care plan, provides ongoing care coordination and treatment planning, tracks and performs required assessments and screening, monitors outcome measures.
Schedule:
Monday through Friday 8:30A.M. – 5:00P.M.
Minimum Requirements:
Bachelor’s Degree in relevant human services field or Associate’s Degree in relevant human services field plus two years of relevant experience.
Salary Range:
$19.00 – $21.50 per hour (depending on experience)
